Waikoloa Restaurant & Bar received a red placard on Sept. 2, 2026. The food establishment, located at 68-1790 Melia St. in Waikoloa Village, is operated by Ofelia Shively.
DOH required the food establishment to take the following corrective actions:

- Points of entry on perimeter walls and ceiling must be sealed, torn window screens to be replaced or patched, kitchen doors must remain closed when not in use;
- Facility must be thoroughly cleaned and contaminated surfaces are to be disinfected;
- The contracted professional pest control operator (PCO) must assess/treat the facility for rodents.
- During the follow-up inspection, the DOH inspector determined that all corrective actions had been completed.
