DCCA DISCIPLINARY ACTIONS

HONOLULU – The state Department of Commerce and Consumer Affairs (DCCA) and its respective state Boards and Commissions released a summary of disciplinary actions taken on individuals and entities with professional and vocational licenses in Hawai‘i, through the month of August 2026. These disciplinary actions include dispositions based upon either the results of contested case hearings or settlement agreements submitted by the parties. Respondents enter into settlement agreements as a compromise to claims and to conserve on the expenses of proceeding with an administrative hearing.

The DCCA and the Boards and Commissions are responsible for ensuring those with professional and vocational licenses are performing up to the standards prescribed by state law.

BOARD OF PHYSICAL THERAPY

Respondent:     Kira C. Markus

Case Number:   PTS 2026-0010-L

Sanction:            $500 fine, provide proof of continuing competence

Effective Date: 7-6-26

RICO alleges that Respondent failed to submit documentation demonstrating compliance with the continuing competence requirements for the January 1, 2023 to December 31, 2024 licensing period, in potential violation of HRS §§ 461J-10.12, 461J-12(a)(7) and/or 461J-12(a)(11). (Board approved Settlement Agreement.)

Respondent:     Michael Takamasa Arakaki

Case Number:   PTS 2026-2-L

Sanction:            $500 fine, complete continuing competence credits

Effective Date:  7-6-26

RICO alleges that Respondent did not complete all continuing competence requirements during the licensing period beginning January 1, 2023 and ending December 31, 2024, in potential violation of HRS §§ 461J-10.1(a). Board approved Settlement Agreement.)

CONTRACTORS LICENSE BOARD

Respondents:   Hawaiian Isle Electric LLC dba HIE Engineering and Jonathan Stern

Case Number:   CLB 2026-138-L

Sanction:            $500 fine

Effective Date:  8-28-26

RICO alleges that Respondent was disciplined by the Department of Labor and Industrial Relations and failed to report the disciplinary action to the Board within 30 days, in potential violation of HRS § 436B-16(a). (Board approved Settlement Agreement.)

Respondents:   Pueo Plumbing LLC and Joshua Stamm

Case Number:   CLB 2026-136-L

Sanction:            $500 fine

Effective Date:  8-28-26

RICO alleges that Respondent was disciplined by the Department of Labor and Industrial Relations and failed to report the disciplinary action to the Board within 30 days, in potential violation of HRS § 436B-16(a). (Board approved Settlement Agreement.)

Respondents:   Joshua’s Electrical Services LLC and Marc B. Bantolina, RME

Case Number:   CLB 2026-0071-L

Sanction:            $5,000 fine, comply with DLIR Decision and DLIR Promissory Note

Effective Date:  8-28-26

RICO alleges that Respondent was disciplined by the Department of Labor and Industrial Relations (“DLIR”) and failed to report the disciplinary action to the Board within 30 days, in potential violation of HRS §§ 436B-16(a), 444-17(6) and 444-17(12). (Board approved Settlement Agreement.)

MIDWIVES PROGRAM

Respondent:     Nichole R. Calagias

Case Number:   MWP 2024-3-L

Sanction:            Probation

Effective Date:  8-27-26

RICO alleges that Respondent was disciplined by the state of Colorado, in potential violation of HRS § 457J-12(14). (Director approved Settlement Agreement.)
